Volunteer Waiver
Terms and Conditions
1. Acknowledgement of Risk:
By volunteering at the 1st Annual Affordable Housing & Resource Fair, I understand that I may be exposed to risks and hazards, including, but not limited to, physical activity, interacting with a diverse population, and other unforeseen risks associated with the event. I acknowledge that I am voluntarily participating and assume full responsibility for any risks of injury, illness, or damage resulting from my involvement.
2. Release of Liability:
In consideration of being allowed to volunteer at the event, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Group Home Alliance, Inc., Harbor Halo, the City of Atlanta, and any of their directors, officers, employees, agents, and representatives from any and all claims, liabilities, damages, or expenses arising from my participation in the event, including personal injury or property damage.
3. Medical Treatment:
In the event of an emergency, I consent to any necessary medical treatment that may be required as a result of an accident or injury during my participation. I understand that I am responsible for any costs related to my medical care.
4. Photo and Media Release:
I hereby grant Group Home Alliance, Inc., Harbor Halo, and the City of Atlanta permission to use photographs, video recordings, or other media in which I may appear for promotional, educational, or other purposes related to the event. I understand that I will not receive any compensation for using such media.
5. Code of Conduct:
I agree to conduct myself respectfully, professionally, and responsibly while volunteering. I will comply with all event rules and instructions provided by the event organizers. I understand that failure to adhere to the event's code of conduct may result in my removal from the event.
6. Confidentiality:
As a volunteer, I may have access to sensitive or confidential information. I agree to keep such information confidential and not disclose it to any third party without prior authorization.
7. Agreement By submitting this form:
I affirm that I have read, understood, and agree to the above waiver terms and conditions. I acknowledge that I am volunteering at my own risk and agree to abide by all rules and instructions provided by the event organizers.
